Referral Process
Our intake workflow is designed to minimize administrative burden on your practice while ensuring patients receive timely, coordinated care:
- Referral Received — Your referral is received and processed by our intake team, same day. Phone and email referrals are both accepted.
- Patient Contacted Within 24 Hours — Our clinical staff reaches out to the patient or their caregiver to introduce our services and confirm eligibility and care needs.
- Initial Home Assessment Scheduled — A qualified clinician visits the patient in their home to conduct a comprehensive assessment, including functional status, medication review, fall risk, and caregiver support evaluation.
- Individualized Care Plan Developed — A care plan is developed based on the assessment findings and your clinical orders, with input coordinated with your practice as appropriate.
- Ongoing Communication with Referring Physician — We provide regular progress updates and communicate any significant changes in patient status, ensuring continuity of care and alignment with your treatment goals.
Our Home Health Services
All Family Health Care offers a comprehensive range of skilled and supportive home health services. Our multidisciplinary team delivers care in the patient’s home, reducing hospital readmissions and supporting recovery and independence. Services available for referral include:
- Skilled Nursing — Wound care, medication management, IV therapy, disease management education, and post-acute monitoring performed by licensed RNs and LPNs.
- Physical Therapy — Therapeutic exercise, gait training, balance and fall prevention, post-surgical rehabilitation, and strength restoration.
- Occupational Therapy — ADL retraining, adaptive equipment assessment, home safety evaluation, and cognitive rehabilitation to restore functional independence.
- Speech Therapy — Dysphagia evaluation and treatment, communication disorders, and cognitive-linguistic rehabilitation.
- Medical Social Worker — Psychosocial assessment, care coordination, community resource navigation, and discharge planning support.
- Home Health Aide — Personal care assistance, bathing, grooming, and activities of daily living support provided under the supervision of a skilled nurse or therapist.

What to Expect After Referral
We understand that timely follow-through on referrals reflects directly on your practice. All Family Health Care takes that responsibility seriously. Here is what happens after you refer a patient to us:
- Same-Day Processing — All referrals received during business hours are processed the same day. After-hours referrals submitted by phone are logged immediately and processed first thing the following morning.
- Patient Contact Within 24 Hours — Our clinical coordinator contacts the patient or their designated caregiver within 24 hours of referral receipt to introduce our team and confirm next steps.
- Home Assessment at the Patient’s Residence — An initial clinical assessment is conducted in the patient’s home by the appropriate skilled clinician — RN, PT, OT, or SLP — based on your referral orders.
- Individualized Care Plan — We develop a care plan that reflects your clinical orders, the findings of the initial assessment, and the patient’s personal care goals. We align the plan with your treatment objectives from the outset.
- Regular Progress Updates to Referring Physician — Physician communication is a core component of our care model. We provide written progress reports and contact your office directly when there are clinically significant changes in patient status.
- Coordinated Discharge Planning — As patients approach their goals, we coordinate closely with your practice to ensure transitions back to outpatient or self-managed care are safe and well-supported.
