Dorothy Joy Balmaceda, M.D. — Administrator
Dr. Dorothy Joy Balmaceda, M.D., serves as Administrator of All Family Health Care and brings deep expertise in the home health industry to her leadership role. A graduate of Doctor of Medicine, Dr. Balmaceda has worked in the home health industry for more than a decade and has served as Administrator of All Family Health Care for over 10 years. Under her leadership, the agency earned a coveted Five-Star Rating in Home Health Compare — a quality recognition from the Centers for Medicare & Medicaid Services that places All Family Health Care among the top-performing home health agencies in the country.
Dr. Balmaceda oversees all agency operations and ensures full compliance with Medicare regulations and Joint Commission accreditation standards. Her medical background and administrative expertise ensure that every patient receives care that is both clinically sound and deeply compassionate.
Rachael Ptaszek, RN, BSN — Agency Supervisor / Clinical Manager
Rachael Ptaszek, RN, BSN, leads our clinical operations as Agency Supervisor and Clinical Manager. A graduate of Resurrection University with a Bachelor of Science in Nursing, Rachael brings extensive experience from subacute and long-term care settings to her role at All Family Health Care. She oversees the development, supervision, and coordination of individualized care plans for every patient we serve — ensuring that each plan reflects the patient’s unique medical needs, personal goals, and physician orders.
Rachael is committed to the highest standards of patient care and works closely with our entire clinical team to maintain consistency, safety, and compassion across all services. Kathleen M. Geary, PT, MBA — Physical Therapist
Kathleen M. Geary, PT, MBA, brings an exceptional combination of clinical expertise and business acumen to our physical therapy program. A graduate of Northwestern University School of Physical Therapy and holder of a Master of Business Administration from DePaul University, Kathleen has amassed over 29 years of orthopedic physical therapy experience — making her one of the most seasoned rehabilitation professionals in the Chicago area.
Kathleen has been a trusted presence in the Edgebrook and Sauganash neighborhoods of Chicago since 2011. Her deep understanding of post-surgical recovery, musculoskeletal rehabilitation, and home-based physical therapy ensures that our patients receive the highest quality care as they work toward restoring their strength, mobility, and independence.
